information as soon as you no longer need them. Credit card applications
received in the mail, convenience checks from your credit card
companies, insurance forms, medical records, bank statements, and charge
receipts. It is essential that you know the major resources to contact
in case you become a victim of identity theft. When contacting
authorities or financial institutions, be sure to keep a log detailing
the conversation including the date, time, and name and phone number of
the agency contacted. Confirm all conversations in writing, keep copies
of all letters and documents, and send correspondence certified mail
(return receipt requested). Credit Bureaus If you suspect that someone
has applied for and received credit using your personal information,
immediately contact the fraud units of the three major credit reporting
agencies: Experian (formerly TRW at 1-800-682-7654; Equifax at
1-800-525-6285; and Trans Union at 1-800-680-7289. Ask that a fraud
alert be added to your account and add a 100-word victim’s statement to
your report stating that your information was used to apply for credit
fraudulently. Include a phone number where you can be reached to verify
any credit applications. Not only do you need to contact the credit
bureaus to make them aware of the situation, you should also ask for the
names and numbers on the creditors with whom fraudulent accounts have
been opened. Also, ask that they notify anyone who has received a copy
of your report within the past six months to alert them of the erroneous
information. Because simply notifying the bureaus may not entirely stop
new fraudulent accounts from being opened, request to receive copies of
your credit report every three months to verify that it contains the
correct information. Creditors When you receive the names and phone
numbers of creditors who have opened fraudulent accounts using your
personal information, contact them by phone and in writing. They may ask
that you fill out and notarize fraud affidavits, which could become
costly. As law does not require notarization, a written statement and
supporting documentation should be enough. You should also notify your
existing accounts if any fraudulent activity has occurred and get
replacement cards with new account numbers. The old accounts should be
listed as “closed at consumers request” because “card lost or stolen”

information is used to set up a fraudulent account or checks are stolen,

Security Administration. You may change your number as a last resort, if

your Earnings and Benefits Statement to check it for accuracy. It is a
good idea to do this once every three years as a precaution. The number

state office of the Department of Motor Vehicles and request to know if
